Understanding Your Explanation of Benefits (EOB)

It’s Not a Bill — It’s a Summary

An Explanation of Benefits (EOB) is a statement your insurance plan sends after a claim is processed, showing what was billed, what the plan paid, and what — if anything — you may owe. If you’re on a Medicare Advantage or Part D plan, this is the document you’ll receive instead of Original Medicare’s quarterly Medicare Summary Notice.

The Key Sections to Understand

  • Provider and date of service — confirms who billed and when.
  • Amount billed — what the provider charged.
  • Amount allowed — the negotiated or approved rate your plan actually recognizes, which is often lower than the billed amount.
  • Amount paid by your plan — what your insurance actually covered.
  • Your responsibility — what you may owe, based on your copay, coinsurance, or deductible status.

Why “Amount Billed” Isn’t What You Owe

One of the most common points of confusion is assuming the “amount billed” figure is your bill. It almost never is — that’s the provider’s initial charge before your plan’s negotiated rate and coverage are applied. The “your responsibility” line is the number that actually matters.

Cross-Check Against What You Remember

Just like a Medicare Summary Notice, an EOB is worth comparing against your own memory of the visit or service — the provider, the date, and the type of service. Catching a discrepancy early is much easier than untangling it months later.

If a Claim Was Denied

An EOB will note if a claim was denied and generally include a reason code. If you believe a denial was made in error, your plan has an appeals process — the EOB itself typically outlines the steps and deadline for filing an appeal.

Keep Them for Your Records

Holding onto EOBs for at least a year gives you a paper trail if a billing dispute comes up later, and it’s useful context if you’re comparing plans during the next Annual Enrollment Period and want a realistic sense of your actual healthcare costs.
