2. Preferred Provider Organization (PPO) Plans

3. Private Fee-for-Service (PFFS) Plans
PFFS plans allow you to visit any Medicare-approved provider who agrees to the plan’s terms. These plans determine how much they’ll pay providers and how much you'll pay for services.
