Part D Drug Changes for 2027

Why Part D Changes Every Year

Medicare Part D plans aren’t static. Formularies, tiers, and cost-sharing structures are reviewed and often revised annually, which means the plan that fit your medication list perfectly this year isn’t guaranteed to fit it the same way next year. As Plan Year 2027 approaches, it’s worth understanding both what typically changes and what’s still unsettled.

What We Know Changes Every Year

Some Part D figures update annually based on federal formulas. For 2026, the standard deductible is $615 and the annual out-of-pocket threshold is $2,100. For 2027, CMS has set the defined standard deductible at $700 and the annual out-of-pocket threshold at $2,400. A plan’s actual premiums, cost-sharing, and coverage details can differ, so check the specific plan’s materials.

These are federal defined-standard-benefit parameters, not a quote of what every plan member will pay. Plan designs, premiums, formularies, and cost-sharing vary. Review your Annual Notice of Change (ANOC) and the plan’s 2027 documents for the coverage you actually have.

What Actually Affects You More: Your Plan’s Formulary

Even when overall program figures stay relatively stable, the plan-specific formulary is where most people notice a real difference. A medication that sat in a low copay tier this year can move to a higher tier, gain a new prior authorization requirement, or be dropped from your specific plan’s formulary altogether for the next plan year. This is exactly the kind of change your Annual Notice of Change is designed to flag — see our guide on reading your ANOC before AEP 2026 for how to catch it.

How to Check Your Medications Against Next Year’s Formulary

  1. Pull your current list of medications, including dosage and frequency.
  2. Compare it against your plan’s 2027 formulary and Summary of Benefits in the carrier’s current materials or Medicare Plan Finder.
  3. Note any tier changes, new restrictions, or drugs that have been removed.
  4. If something changed unfavorably, compare that plan against others available in your area that cover the same medications more affordably.

Prior Authorization and Step Therapy

Beyond pricing tiers, watch for new utilization management rules attached to your medications — prior authorization (your doctor has to get approval before the plan covers it) or step therapy (you may have to try a lower-cost alternative first). These requirements can appear or disappear from year to year even when the drug itself stays on the formulary.

If You’re Comparing Plans During AEP

If your Part D coverage changes for the worse, AEP (October 15–December 7) is your opportunity to switch to a plan that covers your medications more favorably for 2027. Our AEP 2026 checklist walks through the full comparison process, and our guide on choosing the right Part D drug plan goes deeper into the mechanics of comparing formularies.

Check the Official 2027 Parameters

CMS’s 2027 defined standard benefit parameters are a $700 deductible and a $2,400 annual out-of-pocket threshold. These figures do not replace checking the plan-specific premium, formulary, pharmacy network, and cost-sharing. See the CMS 2027 Rate Announcement for the official parameters, and review your ANOC and current plan documents for details that apply to you.

Getting Help With the Details

Comparing formularies medication by medication is tedious, and it’s easy to miss a tier change buried in a hundred-page document. A licensed agent can walk through your specific medication list against available 2027 plans with you by phone, so you’re comparing real numbers instead of guessing.
